I was very surprised to know how many people actually knew what this one meant. A "Dirty Sanchez" is when during sexual activity, the giving partner inserts their finger into the receiving partner's anal cavity. The giver continues by removing the finger and smearing the feces residue onto the upper and lower lip of the receiver to form a mustache disguise similar to the graphic picture on the shirt front.
